Your formal email has four parts.

| Не могли́ бы вы сообщи́ть, когда́ спра́вка бу́дет гото́ва? | Could you let me know when the certificate will be ready? |

The names are invented. Swap in your own request; every line uses this month's Russian.

Each breaks one rule: прошу́ вас, с + the instrumental, нужна́ for a feminine noun, the order of не могли́ бы вы, and рожде́ния.
